It has always been PRN’s mission to assist clients with their loved one’s care and management. The needs and complexity of navigating their loved one’s care have increasingly required more in-depth assessment and care. With the demands of an aging population that wants to remain at home, families spread out over multiple states, and the complexity of the medical and health environment, many reach out to PRN for personalized, in-depth care and management for their loved ones.
PRN can provide the service of a Nurse Care Manager to assist and help guide families through the care and resources available to their loved ones. The main goal of care coordination is to meet clients’ needs and preferences in the delivery of high-quality, high-value health care.
Nurse Care Managers provide care coordination to clients on an individual basis. The Nurse Care Manager will meet with the family and client to assess the needs and goals. This information is used to guide the delivery of safe, appropriate, and effective care.
Nurse Care Management Services
- Oversee, organize and advocate for the best possible client care
- Meets with client and family to determine the client’s needs and goals
- Performs an initial, detailed nursing assessment
- Provides quick availability for emerging or crises situations
- Recognizes and respects client’s goals and self-management abilities
- Develops an ongoing, dedicated care plan
- Coordinates care with families, doctors, home health, hospice, and home care agencies
- Links client to appropriate community resources
- Guides client through complex problem solving
- Assists with medication management
- Escorts client to doctors and medical appointments
- Responds to changes in the client’s condition
- Provides regular assessments and follow-ups
